ICS Role Overview
The Integrated Care Services (ICS) team serves as the central coordination hub that connects patients with healthcare providers, support services, and care resources. Acting as a bridge between patients and the healthcare system, ICS helps ensure timely communication, efficient scheduling, and continuity of care throughout the patient journey.
Key responsibilities of the ICS role may include:
- Coordinating appointments, referrals, and follow-up care
